We are looking for an Assistant Care Manager to join our Learning Disability Transition Team. We are a dedicated team working alongside the Community Learning Disability Teams and form part of the overall County Learning Disability Service in West Sussex.
Are you;

Looking for a challenge and opportunity to develop your skills?

An effective communicator and able to positively engage with people with learning disabilities and their carers / support networks as well as other professionals?

Able to work closely with colleagues to assess need and risk, and plan for appropriate support services?

Experienced / interested in working in social care settings and working with individuals with complex needs?

Passionate about wanting to help individuals maximise their wellbeing and achieve their desired outcomes?

Competent at using IT systems?

You will be based in Horsham or Worthing but may be asked to work across West Sussex. This will include locations not easily accessible by public transport, therefore a full, valid driving license is required, and access to a vehicle for business use.
