West Sussex County Council Learning Disability Service. DO YOU WANT TO HELP US MAKE A DIFFERENCE? We have an exciting opportunity for an experienced Senior Practitioner to join the Challenging Behaviour Commissioning and Support Team based in Horsham.  The CBCST provides a specialist service for adults with learning disabilities and complex needs / behaviour described as challenging.  We are an integrated team consisting of highly motivated and committed health and social care professionals.

The team covers the whole of West Sussex and also manages the care for people living out of area. The post holder will need to:

• Have good skills in supporting a team to deliver effective outcomes for customers with complex needs.

• Have good skills in assessment of need and risk management.

• Have a good understanding of Self Directed Support processes and how they would be implemented for adults with learning disabilities.

• Understand the social context/response to adults with learning disabilities and how this impacts on their support needs.

• Be good at communicating with colleagues, professionals, adults with learning disabilities and their carers.

• Be committed to partnership and team working.

• Understand the need for achieving value for money and be able to achieve this in work with individuals and families.

• Have good Basic IT skills

An important part of your role will be to oversee the care of people we may have who are staying in in-patient settings and people with learning disabilities who may have offending or forensic needs. You will be required to travel across West Sussex and out of county as necessary. This will include locations not easily accessible by public transport, therefore a full, valid driving license is required, and access to a vehicle for business use.

We are looking for an individual with several years post qualifying social work experience. Consideration will also be given to applicants who have an OT or nursing background.
