Salary – £27668 to £33437 depending on qualification and experience

We have an opportunity for a registered Social Worker or Care Manager (registered LD Nurse) to join the Community Learning Disability Service in the Transition Team. You will be located within the Community Learning Disability Team in Bognor and work closely with colleagues to assess need and plan for appropriate support services.

You will have a sound knowledge of relevant legislation and issues affecting young people in transition to adult service; and a good understanding of legislation and policy related to supporting people with complex behaviour needs and its impact on their lives and professional practice.

With skills in assessment of need and risk management, and a commitment to partnership working, you will effectively communicate with young people and adults with learning disabilities, their carers and professionals to progress appropriate support packages using the Self Direct Support model.

You will have relevant learning disability experience in either (older) children or adult work and experience of working with individuals with complex needs is desirable.

This role is county wide and you will be required to travel across West Sussex and out of county as necessary. This will include locations not easily accessible by public transport, therefore an independent means of transport is required, and access to a vehicle for business use.
