We are offering the opportunity to be part of a new team working within a new and innovative service providing Health Coaching for people with long term health conditions. The service, which is being evaluated nationally, will help people to take control in improving their quality of life.

Working with West Sussex County Council and Crawley, Horsham and Mid Sussex

Clinical Commissioning Groups, we are looking to appoint a Health Coach:

37 hours per week, starting £20,661 per annum

Based in Horsham and working in partnership with Health and Social Care providers, you will offer telephone-based help and support that empowers people to make informed decisions and improve their knowledge, skills and confidence in self-management.

All applicants will be able to demonstrate:

• An understanding of long term health conditions and their effects on people's lives
• Experience of working within multi-agency environments
•  Experience of using motivational approaches to help people to improve their quality of life
• Excellent communication skills
• A strong knowledge of support services and facilities within the working area (Crawley, Horsham and Mid Sussex)
