An interesting opportunity is available for a Social Worker or Care Manager to join the Coastal Community Learning Disability Team based in Worthing. Applications from qualified Social Workers, Occupational Therapists or Community Nurses will be considered. The post requires you to contribute to the team’s work of assessment, conducting reviews, writing support plans and delivering a person centred service to individuals with a learning disability. A good understanding of self-directed support and the personalisation agenda will be important within this role. You will also contribute towards the team’s safeguarding of adults at risk through investigation.

You will need to identify eligible needs and ensure support packages address these through quality support plans detailing expectations. You will have excellent communication skills and be able to bring your professionalism to bear upon the work you are allocated. You will join a high performing and supportive team.

The post will require travel throughout Sussex and sometimes further. It is preferable that the successful candidate is a driver with access to a vehicle.
